Mentor Community Services, a part of the Sevita family, believes children belong in the family home whenever it is safe and possible. Through our Family Support Services, we help families of youth at risk with complex mental health, behavioral and other needs overcome the challenges they face together. Our diverse, multilingual specialists support families from a wide range of backgrounds who may be facing out of home placements or reunification.Children Family Specialist Do you have a background providing support in a human services field, and want rewarding work in an organization dedicated to making a positive impact in the lives of others? Salary: $49,000 annually (salary plus overtime) Long term temporary to permanent position Provide in-home intervention for families and children. Assist the families and children to establish weekly goals and help them to identify concrete action steps to reach such goals. Make referrals to services and resources on behalf of the families and children. Attend weekly case reviews with supervisor and clinician. Document all services provided as prescribed by the company, relevant government agencies, and Medicaid Rehabilitative Services policies. Attend all ISPs and IEPs for the families and children. Provide crisis intervention services as needed to prevent placement disruptions. Assess the home environment for safety issues in preparation for a child’s return and following a child’s reunification. Make recommendations regarding family’s readiness for reunification. Conduct diligent search for family resources for a child who is ready to step-down. Qualifications: Bachelor’s degree in a human services field. Two years of related experience. Self-motivated and detail-oriented with ability to multi-task. Strong analytical reasoning skills with ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to maintain composure and exercise sound judgment and discretion. Exceptional communication skills with an ability to establish trust and rapport quickly.
