We want your knowledge and expertise!Job Summary and QualificationsThe Licensed Vocational Nurse/Licensed Practical Nurse (LVN/LPN) coordinates and delivers high quality, patient-centered care by performing a variety of delegated basic patient care services. In collaboration with the RN and other members of the care team, the LVN/LPN provides individualized, comprehensive, and compassionate care in accordance with the nature and specific requirements of the department, and consistent with the scope and standards of practice for the relevant state and specialty.? Obtains and records initial and ongoing patient information throughout the cycle of careProvides direct, evidence-based nursing care of assigned patients as directed by the physician or RNAssists with procedures, monitoring, or other functions as directedPerforms lab and procedural tests within the LVN/LPN scope of practice, after demonstrating competencyInserts and maintains IVs, after meeting training requirements and demonstrating competencyAdministers prescribed medications to patients in accordance with hospital policies and practice guidelines; monitors patient for therapeutic response, notifies provider and intervenes as appropriate in the event of an unintended response to medication Identifies potential nursing problems/changes in patient status, informing appropriate team membersEscalates situations requiring care beyond the scope or practice of an LVN/LPNReinforces education for patient and family based on the individual needs of the customerDocuments the administration of care in the patient medical record in a timely and thorough mannerDemonstrates an understanding of and adherence to the principles of patient privacy and security of protected health informationProvides for the smooth processing of patients into and out of the departmentProvides accurate and pertinent patient information reports to the RN or CNC and to the oncoming shift to ensure continuity of careServes as a resource to department staff, students, volunteers and other departments, and contributes to the teamwork essential to ensuring quality patient careProvides and ensures patient safety at all timesFacilitates the management of the day’s scheduled casesDemonstrates caring and empathy in all interactions and advocates for patient- and family-centered approach in all decisions and actionsDemonstrates excellent communication skills, including active listening and ensures patients, physicians, and colleagues have a clear understanding of all communications, and escalates care concerns appropriatelyParticipates as an engaged, active member of the health care team and positively promotes the rest of the team to patients/families/caregivers and other colleaguesParticipates in professional practice activities and advocates for shared governance functions Seeks opportunities to impact the growth of the organization by promoting excellence in patient experience and providing high quality care to the patients servedEncourages physician engagement by nurturing a positive, professional work environmentFunctions as a good steward of financial resources by effectively managing supplies, equipment, length of stay, and throughput.
