To provide supervision and direction to the Physical Therapist Assistant (PTA) providing care in the certified home care. This position reports to the Clinical/Therapy Manager   Essential Functions: ▪ Conducts initial OASIS and ongoing assessment of the physical, psychological and environmental needs of the patient at the start of care, initial PT evaluation and during each visit ▪ Acts as a case manager and determines the needs for other services, notifies Clinical/Therapy Manager, team members and Scheduler of the patient’s needs ▪ Completes PTA supervisory visits per agency policy/Medicare regulations ▪ Provides full report to clinical manager and appropriate clinical staff to ensure services are provided as needed. Monitors insurance authorizations as appropriate ▪ Participates in mandatory meetings, in-services and case conferences as required by HomeCare Maryland’s policy ▪ Completes all documentation per HomeCare Maryland’s policy, state and Medicare regulations ▪ Maintains and reviews medical records, provides care based on established plan of care and physician orders ▪ Conducts the admission/recertification/resumption of care comprehensive assessment, with OASIS data items and medication review ▪ Adheres to HomeCare Maryland’s policies and procedures ▪ Works in a professional and collaborative manner with physicians, team members and office staff ▪ Ongoing interdisciplinary assessment of the patient ▪ Development and evaluation of the plan of care in partnership with the patient, representative (if any) and caregivers   Requirements   ▪ Current and valid Maryland Physical Therapy license ▪ Current American Heart Association BLS for Healthcare provider certification ▪ Graduate from an accredited Physical Therapy program ▪ Prior experience in home care preferred ▪ Prior managerial experience preferred ▪ 1-3 years’ experience acute care experience as a physical therapist ▪ Fleet safety requirement and a valid driver’s license and auto insurance – travel required Additional Information Who We Are: LifeBridge Health is a dynamic, purpose-driven health system redefining care delivery across the mid-Atlantic and beyond, anchored by our mission to “improve the health of people in the communities we serve.” Join us to advance health access, elevate patient experiences, and contribute to a system that values bold ideas and community-centered care.
