The successful candidate will support New Castle County, DE working out of our North Wilmington Office; work day shift hours 8am-4:30pm, and every other weekend day, some holidays may be required based upon departmental needs.Duties and Responsibilities:Develop Relationships with referral sources, providers and other health care professionals.Pre-admission evaluation and education of patients identified for home health to identify care needs, available resources, and validate information needed to ensure safe transfers across the care continuum.Perform ICD-10 coding of referrals.Coordinate with branch office manager to schedule start of care and supplies for services ordered by the provider as well as communication with referral sources and providers.Identify patients appropriate for specialty programs and telehealth.Requirements:LPNPrevious case management or discharge planning preferred.About ChristianaCare:ChristianaCare is one of the country's most dynamic health systems, centered on improving health outcomes, making high-quality care more accessible and lowering health care costs.
