Job DescriptionThe Genesis Care Manager will work as part of a specialized team within the Primary Care setting of Sun River Health to provide medical case management services to persons living with HIV and/or Hep C. Services include but are not limited to: assessing patient needs; service plan development; care coordination; referrals and linkages; and education and support. Staff will utilize motivational interviewing and a harm reduction approach to provide trauma informed care based supportive interventions.
