Our Rehabilitation Department is seeking a part time, fourteen (14) hours per week, Social Services Assistant. QUALIFICATIONS: Associate degree from an accredited college in Human Services related field RESPONSIBILITIES: Assesses the needs of new clients and devises a plan to assist in meeting these needs Makes referrals for adjunct services, completes applications for entitlements, assists with accessing community resources Outreaches to social service agencies, housing agencies and other support services that may be needed by clients Answers client phone inquiries and provides information and referral Facilitates a meeting with NYSCB counselor and client after assessment is completed to review goals for services and documents outcome Enters assessment and progress reports in the NYSCB Consumer Information System database Performs miscellaneous duties as assigned by supervisor.
