Position Summary/Department Summary:Boston Children's at Martha Eliot provides primary and preventive care for children, adolescents, and young adults, including well-child appointments and sick visits. This full-time access care coordinator clinical social worker will support the behavioral health walk- in clinic in primary care @ Martha Eliot Health Center in Jamaica Plain, MA. (free parking!)Key Responsibilities:Assessments and referrals, providing holding of patients on a waiting list, helping facilitate a waiting list groupProviding brief treatment as well as support other care coordination activities.Carrying an ongoing individual and possible group therapy caseload within the behavioral health department Minimum QualificationsEducation:Master’s degree in social workExperience:Flexibility and ability to work in a fast-paced multidisciplinary team environment as well as excellent communication skills. Experience in a fast-paced community-based health care setting preferredBehavioral health experience with children, adolescents, and families idealBilingual/bicultural/diverse candidates are encouraged to apply.
