Responsibilities include supervising staff, coordinating patient care, maintaining compliance, and supporting clinical excellence.Key Responsibilities:Care & OversightDevelop and oversee care plans from admission to dischargeConduct in-home supervisory visits and provide direct care as neededEnsure timely documentation and communicate care updatesTeam SupervisionTrain, mentor, and evaluate clinical staffParticipate in hiring and performance managementSupport a positive and growth-oriented team environmentCompliance & QualityEnsure regulatory and accreditation compliance (e.g., CHAP)Monitor service quality and implement improvementsPromote safety and ethical care practicesOperational SupportParticipate in on-call rotationCollaborate with care teams and external partnersMaintain equipment and medical suppliesQualifications:RN License (active in applicable state)Associate’s, Diploma, or Bachelor’s in NursingValid driver’s license and acceptable MVRClinical experience per state/federal guidelinesPreferred: Home health or private duty leadership experienceSkills:Strong clinical judgment and communicationLeadership and team developmentKnowledge of home care regulations and documentationWork Location:Wilkes-Barre PA – In-home visits and potential hybrid (based on branch need)Physical & Work Requirements:Ability to lift up to 40 lbs; walk, bend, kneel, and climb stairsWork in varied home settings; potential exposure to allergens and infectious diseasesAdditional Information:This job description is not all-inclusive and may evolve as organizational needs change.As a Medicare/Medicaid provider, employees must meet all required health and vaccination standards, subject to applicable medical or religious exemptions.As an employer accepting Medicare and Medicaid funds, employees must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate.Notice for Job Applicants Residing in California
