A growing company with opportunities for advancement Company sponsored training, tuition reimbursement, and other learning opportunities   Job Description The Social Services Assistant is responsible for assisting the Director in planning, developing, organizing, implementing and directing the SNF Health & Wellness in accordance with current existing federal, state and local regulations, as well as Stellar Senior Living established policies and procedures, to ensure medically related social services are provided to all residents in the facility.    Responsibilities Plan, organize, develop the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are met Ensures the quality of social services rendered by the facility at all times Participate in the admission, discharge, MDS coordination, etc for social services Refers patients and families to appropriate community resources as necessary   Qualifications Degree in related field. At least two years of experience in Long Term Care preferred.
