Programs are offered for children, adolescents and adults struggling with a wide range of behavioral health issues.Job SummaryWorks in coordination with interdisciplinary teams to organize, prepare, and deliver evidence-based substance use and addiction treatment services. Maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics.Collaborates with the interdisciplinary team to conduct a comprehensive bio-psychosocial assessment, develops and implements treatment/discharge plan, monitors progress of interventions and revises plan to meet patient and family needs, and coordinates consults/referrals to remove barriers and promote the individualized care plan, detox, and/or substance use rehab.Provides education to patient/family with respect to meeting established goals, including but not limited to disease prevention and progression, advocacy for benefits, and maintenance of treatment regimen.Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, referral and discharge planning.Maintains timely, patient-centered documentation that reflects the patient and client systems’ pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies.Participates in administrative and developmental activities including team meetings and clinical supervision by program manager.Works as a team member to contribute to improved patient experience. Qualifications Master's Degree in Social Work, Counseling, Marriage and Family Therapy or related Human Service fieldPrevious experience in a healthcare setting working with substance use disorders or addiction servicesLCSW, LADC, or LPC; responsible for maintaining current licensureValid Driver's LicenseComputer proficientAbility to assess problems, determine solutions, and make decisions within a timely mannerUtilize supervision and team approaches in the context of patient assessment and clinical interventionsExcellent communication skills, with the ability to establish relationships within and outside their entityFocused on personal, patient/client and unit/staff safety At the Institute of Living, we don’t just take great care of our patients; we take great care of our Clinicians!
