LOCATION: WINTER HAVEN, FL CLIENT: Creative Health Solutions - Astoria Health and Rehab PT  701 Overlook Drive Winter Haven Fl  SETTING: SNF, IP, and OP mixed settings START DATE: July GUARANTEE: 35 hours ASSIGNMENT LENGTH: 13 Weeks REQUESTED EXPERIENCE: Prefer candidates with at least 1 yr. in SNF or Clinical rotation/externship in the SNF. **MUST be strong in SNF caseload and able to work independently *Currently using a traveler but they are not working out because of lack of experience STAFF: Total number of team members 15 and 1 Rehab Tech EMR:  NetHealth / RehabOptima  CASELOAD / PATIENT SCHEDULING: 90% Productivity but client understands caseload may vary and says average is 88% *2 hour orientation and expectation is to hit the ground running and meet productivity SCHEDULE: Mon-Fri ADDITIONAL INFO: 132 Bed facility skilled nursing inpatient and outpatient rehab LOCATION HIGHLIGHTS: Winter Haven is a city in central Florida, known for its canal-linked Chain of Lakes. Several lakes have waterside parks and eateries, as well as boat ramps and water-sports rentals. On Lake Eloise, Legoland Florida Resort includes Cypress Gardens, a 1930s botanical garden, and Lego-brick attractions, rides and a water park.
