The Medication History Technician, under the supervision of a licensed Pharmacist and in accordance with all State and Federal laws, will perform medication history assessments for patients being admitted to the hospital, providing complete information regarding each medication identified (i.e. drug, dose, route, frequency, indication, last dose taken). The technician will verify patient allergy information and ensure accuracy of the prior to admission (PTA) medication list. While adhering to the ICARE principles and exhibiting the AIDET principles, the Medication History Technician will demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.Job Description:Perform Medication History AssessmentActively includes the patient and/or the patient's family whenever possibleDocuments the patient prior to admission medications accurately and completelyContacts the patient's outside pharmacist or physician to resolve all discrepancies in a timely mannerDocuments resolution of discrepancies in the medical record so that the attending physician can review and reconcile the listDocuments two sources used to confirm the accuracy of the patient’s medication listSets priorities in correct order in relation to the needs and circumstances at the time with reference to the patients, co-workers, physicians, and the nursing unit, etc.Provides superior customer service while following the AIDET principlesDocument the accurate medication history and allergy information in the electronic medical record in a timely mannerCollaborates with pharmacy, nursing, providers, and other healthcare staff to assist with system medication history needs as requestedResponds to medication history questions and needs in a timely and courteous mannerPerforms monthly floor checks for ED area drug storage locations and assists with medication delivery to ED areas as requestedPerforms other duties as assignedEducation, Credentials, Licenses: High school graduate                           National Certified (PTCB or NHA) or active intern license  Kentucky Board of Pharmacy technician registrationIndiana Board of Pharmacy technician registration within 90 days of hire                                                          Specialized Knowledge: Excellent verbal and written communication skillsCritical thinking and sound judgmentTrained in general use of pharmacy computer systemsAccurate work with careful attention to detailAbility to prioritize work in a high-volume work environmentKind and Length of Experience: 24 months in an appropriate settingFLSA Status: Non-ExemptRight Career.
