Job Title: Clinical LeaderDepartment: Long Term Care Location: DeMay Living Center Hours Per Week: 40Schedule: Days, 7a-330p Sign-On Bonus: Up to $20,000 SUMMARY:The Clinical Leader is a Nursing Leader, who serves as a resource for problem solving, provides clinical expertise, facilitates staff mentoring and coaching, and facilitates resident flow.RESPONSIBILITIES:Round on staff and physicians.Using unit census, round on all residents daily, using key words at key times to identify issuesAsk residents if there is staff that should be recognizedResolve any issue or concerns at the time of the roundingFollow up any concerns with Nurse ManagerComplete rounding log and turn in to Nurse ManagerIdentify and have knowledge of all high risk residents (including residents in restraints, residents with previous falls or at high risk for falls, residents with skin breakdown, residents with a long LOS, and admissions and transfers to the unit), ensure plan of care is currentRecognize staff publicly, for a job well done or for role modelingWrite thank you notes to staffCoach staff privately when performance is below standardInitiate counseling and/or assist with disciplinary action processMake assignment based on needs of residents and competency of staffEnsure all LPN staff are assigned to an RNEnsure appropriate number of staff available on unit at all times; assign and monitor lunch, break, and committee meeting coverageAssist staff with care as neededCommunicate any staffing overages or needs to Nurse Manager and/or Off Shift DirectorMaintain staffing with in budgeted hours per resident day for censusEvaluate staffing for next shift, day, weekConsistently plan on unexpected admissions and facilitate admissionsAddress issues delaying resident dischargesCommunicate with Clinical Bed Coordination regarding all admissions, transfers, and discharges if appropriateAssist staff in care to expedite flow and bed availabilityFacilitate admissions by accepting admission and transitioning to assigned nurseCoordinate discharges and admissions for the dayEnsure compliance with Resident Safety GoalsEnsure emergency equipment is checked dailyEnsure refrigerator logs are completed dailyEnsure compliance with Infection Control policiesREQUIRED QUALIFICATIONS:Current New York State Registered Nurse LicenseEffective 1/1/2014, BLS required for all new candidates prior to hireBLS required for all internal team members by 12/31/2014PREFERRED QUALIFICATIONS:Associates degree in Nursing required; Bachelor’s of Nursing preferred.2 years RN experience preferredFor Long Term Care: experience with frail, elderly/geriatric population preferredEDUCATION:AS: Nursing (Required)LICENSES / CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA)American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. It may be modified in the future and eligible for additional pay components.
