When you thrive, so does the community of care we’re building together.  Offer Based on Years of Experience What You Need to Know: Be the Best Palliative Clinical Social Worker You Can BeIf you meet these qualifications, we want to meet you!Master’s Degree from accredited school of Social Work or related field; Valid license/registration of permit to practice in state(s) of agency operation if requiredMedicare/Medicaid/Insurance specialty preferredUnless otherwise dictated by the state, at least one (1) year of professional social work experience (post-graduate) in a healthcare setting required; 2+ years preferredExperience or education in grief counseling preferred.  Discharge planning experience preferred.Required Certifications and Licensures:Licensed to practice as a clinical social worker in the state of agency operation if requiredMust be a licensed driver who can travel to all business locationsResponsibilities:   As a Palliative Care Clinical Social Worker, you will: Serves as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of a patient and family in a hospital, home, and post-acute settingProvide counseling to understand disease trajectory towards acceptanceServes as liaison between patient and Palliative Care team and Hospice team to provide continuity of careAssist the core members of the Palliative Care team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spritual well-bing, and human dignity of each patient.Assess the social, spiritual, and emotional needs/factors to estimate the patient's and involved caregiver's capacity and potential to cop with the problems of daily living and with serious illnessSupport the patient and chosen family through education and facilitate the completion of Advanced Directives including funeral planningProvide discharge planning related to change of level-of-careEducation of the patient and family on how to proactively manage symptoms, as well as how to respond appropriately in times of crisis with an emphasis on utilizing palliative in-home support and other lower acuity interventions when appropriate.Our Investment in You  Caring for others starts with caring for you.
