Family Practice / Internal Medicine (Hospitalist) Physician Madison WIUrgently HiringPart time We are looking for an experienced Internal Medicine or Family Practice Physician to join our practice in Madison, WI. We are seeking a compassionate provider to be our go-to person for ensuring the best patient care possible in our Rehab Hospital/care facility. This opportunity is part time, working in a post-acute hospital with higher acuity pts including rehab s/p neuro/trachs/vents, and wound care patients. Rotational schedule with another provider covering 7 on 7 off (or a combination of days split with other providers). Shifts ranging from 5-8 hours (dependent on efficiency).This position offers excellent work-life balance with flexible starting and ending times in lieu of a 12-hr. shift requirement. As census ramps you will have APP support and a scribe to assist with documentation as well as partnership track opportunity. Additionally, we offer a very robust hourly compensation and benefit package. You will experience one of the most collegiate groups of people you will ever work with. Great opportunity for off week rotation. The physician is responsible for diagnosing and treating patients in a patient centered compassionate environment as a part of a cohesive interdisciplinary clinical team.Responsibilities:History and Physical Exams- Examine patients and their medical records, pertinent documents/imaging/labs/testOrder and study diagnostic tests (blood tests, X-rays etc.)Review and approve rehabilitation care plans as needed Actively participate in interdisciplinary team and patient care conferences Diagnosis, management and treatment of acute episodic illnesses and stable chronic conditions, evaluation of current therapies, health maintenance, health promotion, illness prevention education and counseling, and management of urgent care mattersPrescribe medicationsWrite referrals and consult other physicians for comprehensive patient careMaintain accurate records and schedules- Establishes and maintains a system for tracking and coordinating the designated patient caseload and their progress throughout the entire acute or chronic episode of careInform patients about their health conditions and prescribed medicationsEducate patients and their family members on diseases, epidemics and prevention- teaching and counseling to patients and familiesCollaboration with the healthcare team- Monitors, evaluates, and communicates relevant data regarding clinical problems consulting physicians and appropriate members of the interdisciplinary teamOversee patients’ overall health- identify opportunities for each patient's needs relative to his/her age, Monitors progress of patients and collaborates with other team members to ensure continuity of care.Identifies and addresses potential deterrents to compliance with treatment regime.Advance professional knowledge and conduct researchFollows up with patients if they are discharged to assess if needs are being met and determines if additional follow-up care is required.Assure the provision of patient services while on dutyCollaborate closely with the leadership regarding operational policies and procedures that may impact patient careAttend monthly meetings as requiredReview and approve rehabilitation care plans as needed and actively participate in interdisciplinary team and patient care conferences Skills:Valid Medical license in WIMD/DO BCBE Hospitalist (Internal Medicine OR Family Practice/Primary Care with Hospital Medicine experience preferred)Federal DEA and possess or eligible for Controlled Substance Certificate.ACLS CertifiedSchedule:Part timeFlexible shifts in a 7 on 7 off rotation.
