Bilingual Behavioral Health Clinician Department: Diabetes Initiative Schedule: Full-Time Location: Mattapan, MA (Onsite Job) Salary Range: $65k - $80k A well-known, mission-driven health system in Boston is seeking a Behavioral Health Clinician to join their team. This role offers the opportunity to work at the intersection of behavioral health and chronic disease management, with a strong focus on advancing health equity in diverse communities. Position Overview The Behavioral Health Clinician will work closely with patients who have, or are at high risk of developing, cardiometabolic conditions such as diabetes, hypertension, or obesity—alongside associated behavioral health needs including depression and anxiety. As part of a multidisciplinary team, this clinician will deliver trauma-informed, evidence-based care to help patients manage co-occurring health conditions and improve overall outcomes. This is a hybrid role with both in-person and telehealth care, including participation in community-based programming. Key Responsibilities Provide behavioral health assessments, crisis interventions, and short-term therapy (up to 6 months). Develop patient-centered care plans that integrate behavioral health and chronic condition management. Facilitate both individual and group therapy sessions. Collaborate with psychiatrists, psychologists, and medical providers to coordinate care. Monitor patient progress with evidence-based tools (e.g., PHQ-9) and adjust treatment as needed. Participate in community outreach, education, and programming related to behavioral health and diabetes management. Attend interdisciplinary team meetings and contribute to program planning and evaluation. Qualifications Master’s degree in Social Work or Mental Health Counseling (required). Massachusetts licensure as LMHC, LCSW or LICSW (required). Bilingual (Spanish or Haitian Creole) strongly required Strong interpersonal, communication, and cultural competence skills.
