ESSENTIAL JOB FUNCTIONS, DUTIES, AND RESPONSIBILITIES: ·    Review prescriptions issued by prescribers to assure accuracy and determine formulas or ingredients needed. ·    Evaluate prescriber medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Make recommendations for medication therapy changes to prescribers as appropriate. ·    Collaborate with the healthcare team to provide clinical and technical support as needed. ·    Document all communication with physicians, nurses, and other members of the healthcare team. ·    Take responsibility for accurate intake and efficient fulfillment of prescription orders. ·    Obtain, as necessary, and maintain all required documentation in accordance with federal and state regulatory agencies and Pharmacy Services policy. ·    Provide and document interim Medication Regimen Reviews for all new admit or readmit patients. ·    Implement and document all approved therapeutic substitution medication orders. ·    Provide appropriate clinical assessment prior to contacting healthcare center. ·    Demonstrate effective and professional leadership skills. ·    Document and investigate errors and variances. ·    Take ownership and accountability for the successful completion of all pharmacy production duties and achievement of pharmacy location goals.LICENSURE, CERTIFICATION, EXPERIENCE AND EDUCATION REQUIREMENTS:·    Graduate of ACPE accredited School of Pharmacy.·    Current licensure in the state of practice.·    Long-term care pharmacy experience preferred.·    Experience in patient care and medication management, drug therapy monitoring, drug and medical supply procurement, inventory control management, and performance improvement.·    Ability to clearly communicate medication information to professional practitioners and the general public.Family Makes Us Stronger.
