With integrity at the center of all we do, we know that when we support you and your community, you'll change lives every day.As a Clinical Transition Coordinator (CTC), you will: Ensure timely and responsive care by providing daily onsite access to transitional care coordination and ensuring timely assessment of patient and family needs prior to dischargeProvide information to families and patients to manage careSupport the hospital staffExpedite referrals for any type of post-acute care (home care, hospice, palliative, personal care and support services, skilled nursing facilities & rehab)Improve the transition to the home care teamProvide insights on post-acute care trendsTo qualify as a Clinical Transition Coordinator (CTC) with us, you will need: Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN), Registered Nurse (RN), Physical Therapist (PT), Physical Therapist Assistant (PTA), Occupation Therapist (OT), or Certified Occupational Therapist Assistant (COTA) in the state associated with this position requiredOne (1) year of professional experience practicing as listed above, requiredReliable transportation to/from care sites and/or work locationsPrevious experience in home care At Interim HealthCare, we know that being our best is non-negotiable - that's why we treat your family like our own.
