As a Registered Nurse you will:  Key Responsibilities:Assess and evaluate patient needs/problems, identify mutually agreed upon goals for care plan with patientsReport patient status, updates and discharge planning process (if applicable) to clinical leadership, attending physician and hospice physicianAbility to perform an annual on-site visit to the location where a patient is receiving care in order to observe and assess each aideAttend staff meetings and educational in-services as neededBe responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care Skills and Experience Required: A current unencumbered State Professional Nurse LicenseMust maintain a valid driver's license and good driving recordAbility to work in a field setting and exhibited ability to make sound nursing judgmentAbility to assess patient needs and formulate individualized patient care plans to meet those needsExperience with Microsoft Office
