We are seeking a compassionate and driven Social Worker to join our Skilled Nursing team in CO. In this role, you will support residents and families through the continuum of care by providing psychosocial assessments, care planning, discharge coordination, and advocacy services. Working collaboratively with the interdisciplinary team, you will help ensure each resident’s emotional, social, and transitional needs are met in accordance with CMS and Colorado state regulations.
