Your recruiter will provide the most up-to-date details during the hiring process.How you’ll make an impact in this role Lead the multidisciplinary care team as the central coordinator, ensuring every specialist and therapist is aligned to provide seamless, holistic patient care.Execute and adapt personalized care plans by vigilantly monitoring patient progress to ensure every clinical intervention is timely, safe, and effective.Document and communicate patient milestones with precision to ensure the entire medical team has an accurate, high-quality roadmap for the patient's recovery.Empower patients and families for life after discharge by coordinating with the healthcare team to ensure a safe, confident, and well-prepared transition home.What minimum requirements you’ll need Licensure / Certification / Registration:BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
