We’re rapidly expanding and we need great people to join our team.The Supervisor, Specialty RN Nephrology is a multi-market leadership position accountable for the direction, leadership and support of our ChenK nurses, in delivering the day-to-day programmatic specialty care strategy. The incumbent is responsible for clinical oversight of our care coordination efforts across the continuum of care, ensuring that our patients receive and experience the highest quality of care at outpatient, inpatient & post-acute facilities, leveraging all available resources, in line with the ChenMed model and Specialty Team standards of care.This role collaborates closely with market leadership to ensure that the clinical needs of our patients are met in all facility-based healthcare environments with high quality care and proper utilization of healthcare resources.This role collaborates closely with facility-based healthcare providers, dialysis facilities, Nephrologists, Vascular and IR, consultants and our PCP-led care teams to ensure the patient's progression through the continuum of care is effective, efficient, equitable, safe and achieves the desired clinical outcomes with effective resource utilization.ESSENTIAL JOB DUTIES/RESPONSIBILITIES:Supervises a team of nursesProvides day-to-day oversight of staffing/scheduling needs, problem solving, and performance management.Participates in recruitment process and talent management activities.Conducts regular coaching and mentoring sessions.Supports the clinical & non-clinical needs of patients with ESRD or late-stage CKD, in the day-to-day execution of our ChenK standard operating procedures.Monitors performance and delivery of ChenK key performance indicators related to ESRD and late-stage management at the local level.Ensures ChenK nurses understand key performance indicators and have necessary tools and resources to effectively deliver expected outcomes.Coaches the ChenK team on identifying and managing care of the complex CKD/ ESRD population, including medication management, and knowledge deficit identification.Implements coaching at the market level when deficits noted.Ensures team members can identify barriers for safe discharge, coordination with dialysis, transportation, and execute standard operating procedures to achieve readmission avoidance.Provides direct support for care coordination of complex patients in post-acute care facilities in collaboration with post-acute physicians and management on discharge to ensure the hand off to the PCP and the dialysis facility is not a gap in care. Seeks assistance from ChenMed's Specialty and Network Teams, Acute Case Managers and Community Case Managers when needed to support patient care.Assists with implementation of SOPs as requested by management.Participates in quality improvement initiatives and special projects assigned.Provides coverage for PTO and assists with staffing needs as requested.Serves as a preceptor when assigned.Ensure compliance with all operating processes and service excellence standards.Performs other duties as assigned.KNOWLEDGE, SKILLS AND ABILITIES:Knowledge and understanding of ESRD management and late-stage CKD care coordination. Strong interpersonal and communication skills and ability to work effectively with internal and external stakeholdersCritical thinking skills and ability to work with moderate autonomyLeadership skills to guide team members toward objectivesAbility to manage multiple priorities and work in stressful situationsCommitment to maintaining confidentiality and patient care standardsFlexibility with schedule, including off-shifts, weekends and holidaysKnowledge of care coordination standard operating proceduresUnderstanding of case and utilization management procedures in post-acute settingsBasic understanding of healthcare economics and Medicare cost driversUnderstanding of Managed Medicare principlesAbility to assess patient progress and adjust care plansKnowledge of nursing and case management theory and practiceKnowledge of community health services and social service networksOrganizing and coordinating skillsProficiency in Microsoft Office Suite products including Excel, Word, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation softwareAbility to travel locally up to 20% of the timeSpoken and written fluency in English, bilingual preferredRequires use of independent judgmentEDUCATION AND EXPERIENCE CRITERIA:BA/BS degree in Healthcare Administration or related field preferredA valid, active Registered Nurse (RN) license in State of employment plus one of the following: Bachelor's Degree in Nursing (BSN); With historical ESRD work in dialysis or Bachelor's degree in health-related disciplineCompact license required if available in stateMinimum three (3) years in Chen K nurse roleBasic Life Support (BLS) certification required within first 90 daysCurrent, valid Driver's License requiredTraining in operational and quality improvement skills preferredKnowledge of managed care environment requiredPAY RANGE:$83,638 - $119,482 SalaryThe posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors.
