This role assists individuals who are nearing end of life, helping maintain dignity, comfort, and quality of life. The Hospice Aide is an essential member of the care team and delivers care in alignment with the patient’s individualized Plan of Care. How You Will Make an Impact• Provide assistance with at least 2 of the following ADL's including personal hygiene• Assist patients with medication intake and prescribed exercises• Incidental household services, which are essential to patient health care• Will follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visits• Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care The Experience and Skills You BringMust be a certified nurse’s aide (CNA). State Tested Nursing Assistant (STNA) for OhioMust maintain a valid driver's license, good driving record and reliable automobileHigh school graduates or GED preferredDemonstrate the ability to be able to work within a multi-disciplinary team settingDemonstrate the ability to work in field situationsPreferred Knowledge, Skills, and ExperiencePreferred candidates will have in-home and/or hospice experience
