Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.Current Physical Therapist licensure in Illinois required to start the assignment.Current CPR certification required.Must have a valid driver’s license, vehicle insurance, and reliable vehicle for patient home visits.Experience providing home health physical therapy preferred. This includes evaluating functional status, developing and implementing home-based plans of care, and documenting per CMS requirements.Ability to complete evaluations and develop PT plans of care within state-specific guidelines and to report outcomes to the nurse supervisor within 24 hours of evaluation.Schedule flexibility required. Must be able to participate in weekend rotation and potential observed holiday availability as needed.May be required to supervise Home Health Aides and PTAs in accordance with state regulation and agency policy. Previous supervisory experience preferred.
