Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.Active Texas Physical Therapy license is required to start the assignment.BLS Certification required from AHA or ARC.Home Health experience required, preferably in a geriatric patient population.Experience with HCHB (Homecare Homebase) EMR preferred.Ability to manage a weekly patient mileage expectation of approximately 250 miles.Reliable transportation and valid driver's license required for travel throughout the Tyler, Longview, and Marshall territories.Weekend rotation not required. On-call not required. Assignment Details Facility Type: Home Health.Shift Name: Days.Shift Details: 5 days per week.Territory/Coverage Area: Tyler, Longview, Marshall.Expected Mileage Per Week: 250 miles.Floating: Candidate will be based out of Tyler or Longview and may cover the broader territory as needed. Tyler is home to the Tyler Rose Garden and the annual Rose Festival, located about 2 miles from downtown and within the larger Tyler State Park area, providing access to hiking and boating on a nearby reservoir within a 15-minute drive, suited for an outdoor lifestyle. The Tyler Historic District and Brick Streets District provide urban dining, museums such as the Goodman Museum, and a walkable downtown core with local theatres and galleries within a short commute, suited for cultural and leisure activities. For weekend trips, Lake Fork and Lake Palestine are within a 45- to 60-minute drive and offer fishing and water recreation, while the Piney Woods region and nearby state parks provide camping and trails for an adventurous, nature-oriented lifestyle.
