Minimum Requirements Bachelor’s, Master’s or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.Active West Virginia state PT license is required to start the assignment.BLS (Basic Life Support) certification from AHA or ARC may be required.Experience working in skilled nursing or long-term care settings preferred.Proven ability to perform evaluations, develop individualized treatment plans, and document clinical progress in electronic medical records.Client requires candidates who can provide care 5 days per week and deliver consistent caseload management in a skilled nursing environment.Preferred: Experience with restorative nursing programs, fall risk assessment, and skilled rehab documentation standards. Assignment Details Facility Type: Skilled Nursing Facility.Shift Name: Days.Shift Details: Days 0800-1630.Work Days: Monday through Friday.Shift Type: 5x8 schedule.Assignment Length: 14 weeks.
