Tennessee Quality Care is looking for a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team in the Shelby County, TN area. In this role, you will provide skilled intermittent nursing visits—not shift-based or private duty care. You’ll support patients recovering from illness, surgery, or managing chronic conditions by delivering clinical interventions in their homes.As a Home Health LPN, you will conduct focused assessments, carry out the plan of care established by the RN Case Manager, and help patients reach their health goals through education, treatment, and proactive care coordination.
