Minimum Requirements Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent entry-level degree in physical therapy.Current, active Florida Physical Therapist license required to start the assignment.Proof of current CPR certification required.Current driver’s license, reliable transportation, and personal auto insurance required due to home health travel responsibilities.1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.Experience with durable medical equipment and ordering appropriate rehabilitative equipment preferred.Previous home care experience preferred.Ability to perform documentation: prepare and revise Plans of Care, progress notes, phone orders, and other clinical records.Ability to perform clinical assessments, provide neuromuscular, neurological, and orthopedic treatment, and instruct patients and families on transfers and assistive devices.Ability to travel locally and work a flexible schedule. Ability to perform physical demands of the role including lifting, bending, prolonged standing and walking. Shift pattern: 5x8.Candidates must have reliable transportation and be prepared to travel locally to patient homes.
