Summary JOB SUMMARY/PURPOSE Responsible for the coordination of activities involving medication safety in the organization. Provides leadership for medication safety event reporting and response, educates providers and staff on the system-based causes for medication errors, communicates best practices regarding effective medication safety strategies to others within the system and interfaces with hospital site and system pharmacy and quality representatives on medication safety issues. Responsible for review, analysis and trending of all medication safety events for the system (medication errors and adverse drug events (ADEs)) and participation in any root cause analysis involving a medication event as well as creation or review/revision of patient care policies that impact the safe use of medications. Elevates the importance of medication safety at Samaritan. Provides leadership and accountability for compliance with regulatory and accreditation standards set forth by DNV, DEA, OBOP, USP, FDA, and OIG. Leads drug diversion monitoring program in collaboration with site pharmacy managers, nursing and quality leadership. Performs pharmacist staffing responsibilities. DEPARTMENT DESCRIPTION Pharmacy Administration provides leadership and direction to the various pharmacy departments and programs at SHS. These include inpatient and outpatient pharmacies, Medication Management and pharmacy residency programs as well as 340B operations. We are staffed with seasoned pharmacists and technicians who help optimize pharmaceutical care throughout our SHS service area. EXPERIENCE/EDUCATION/QUALIFICATIONS Current unencumbered Oregon Pharmacist Licensure required. One (1) of the following required: Satisfactory completion of an ASHP-accredited PGY2 Health System Pharmacy Administration & Leadership Residency. Satisfactory completion of an ASHP-accredited residency in pharmacy practice plus one (1) year clinical practice experience in hospital or health system pharmacy practice. Two (2) years clinical practice experience in hospital or health system pharmacy practice. Pharmacist In Charge experience preferred. Completion of an ASHP Accredited Pharmacy Residency Program preferred. All job offers are contingent upon the candidate passing a pre-employment/post offer drug screen.
