Minimum Requirements Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA.Current, unrestricted West Virginia physical therapy license required prior to start.BLS certification from AHA or ARC may be required.Experience with OASIS visits and add-on evaluations required.Home health experience preferred. Ability to complete documentation in a timely manner and meet productivity expectations (points-based productivity may apply). Assignment Details Facility type: Home Health.Shift: Days.Shift hours: Monday–Friday, 08:00–16:00.Weekly guaranteed hours: 40.Coverage area: Kanawha County; local travel between patient visits required. Mileage reimbursement between first and last patient is provided.On-call/weekend requirement: None.Dress code: Royal blue scrubs.Must support community- and state-focused patient populations.
