We're hiring a licensed pharmacist for a contract position in Tulsa, OK at a hospital. You will use clinical expertise to ensure the safe and accurate distribution of medications and provide clinical pharmacy services across inpatient care areas. Minimum Requirements: Active Oklahoma Pharmacist license required.Experience working as a pharmacist in an acute care hospital setting required.EPIC electronic health record experience required.Experience with medication reconciliation for discharge medication lists required.Experience with antimicrobial stewardship, including interpretation of culture results and making recommendations for UTI, pneumonia, SSTI, and similar infections preferred.Experience with pharmacokinetic dosing management and anticoagulation dosing management/monitoring preferred.Ability to prepare, sterilize, and label parenteral medications and to maintain sterile product labeling and dating as required.Knowledge of controlled substance handling, documentation, and recordkeeping as required by law.Strong communication skills for gathering home medication histories and providing discharge counseling to patients and families required.Minimum 6 months pharmacy experience preferred. Assignment Details Facility Type: Hospital.Shift: Day (5x8).Duration: 13 weeks.Must follow facility pharmacy policies and meet federal and state pharmacy regulations.Participation in pharmacy quality assurance, Pharmacy & Therapeutics Committee activities, and medication use review required.EPIC responsibilities include medication reconciliation and documentation of charges and billing functions. Standard Job Description: Work under the general supervision of the Pharmacy Director. Embrace the organization’s core values and contribute to a culture that prioritizes patient and employee safety. Maintain adequate drug supplies and follow established procurement and inventory procedures. Prepare, sterilize, and label parenteral medications and ensure competency of personnel preparing high-volume parenterals. Dispense and document medications accurately, including stat and floor stock orders. Maintain cleanliness and temperature monitoring in pharmacy areas. Conduct monthly inspections of patient care units where medications are stored or administered. Ensure proper control and documentation of controlled substances. Participate in development and maintenance of the hospital formulary and provide clinical consultation to prescribers and staff. Provide patient education and discharge counseling. Participate in quality assurance activities related to drug utilization and effectiveness. Support antimicrobial stewardship and perform pharmacokinetic and anticoagulation monitoring as required. Participate in employee training, orientation, and professional education. Perform other duties as assigned.
