Minimum Requirements Physical Therapist degree required (PT).Active Pennsylvania Physical Therapy license required to start the assignment.BLS certification from AHA or ARC may be required.Experience in home health settings preferred.Experience performing evaluations, start of cares, OASIS visits, and return/supervisor visits preferred.Ability to travel within and cover assigned counties (Somerset County, Cambria County, Northern Cambria, Indiana County).Must follow facility dress code: royal blue or black scrubs, or khakis with a polo shirt. Assignment Details Facility Type: Home Health.Shift: Days.Shift Hours: 08:00 - 16:00.Weekly Hour Guarantee: 40 hours (32 hours guaranteed noted by client; weekly guarantee listed as 40 hours).Productivity expectation: 30 points.On-call/Weekend: Weekends and on-call requirements vary; discussed with client. Every 8 weekends rotation may apply.Coverage Area: Somerset County, Cambria County, Northern Cambria, and Indiana County. Mileage reimbursed between visits.Assignment Type: Travel. The area offers outdoor recreation, scenic byways, and reasonable commute times to regional medical centers and transit corridors connecting to I-70 and PA state routes for regional travel.
