Minimum Requirements Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.Active Indiana Physical Therapy license required to start the assignment.BLS certification from AHA or ARC may be required.Experience working in skilled nursing or long-term care settings preferred.Experience with mobility assessment, therapeutic exercise, gait training, and restorative nursing programs preferred.Ability to document patient care in electronic medical records required.
