Minimum Requirements Degree in Physical Therapy (Bachelor's, Master's, or Doctoral) from an accredited program approved by APTA.Active California Physical Therapist license required to start the assignment.Current BLS (Basic Life Support) certification from AHA or ARC may be required.Home health experience preferred. Candidates with experience in skilled nursing, outpatient orthopedic settings, or hospital acute care are encouraged to apply.Ability to perform patient evaluations, develop and implement individualized treatment plans, document progress according to state and agency standards, and coordinate with interdisciplinary teams.Valid driver’s license and reliable transportation required for home visits.Electronic documentation experience preferred, including familiarity with common home health EMR systems.
