Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.Active Wisconsin Physical Therapist license required to start the assignment.Current BLS certification from AHA or ARC may be required.Experience working in skilled nursing facility settings preferred.Demonstrated ability to document skilled evaluations, plan of care, and skilled progress notes in an electronic medical record preferred.Ability to work Monday through Friday schedule and meet a 35-hour weekly commitment as outlined by the client preferred. Assignment Details Facility Type: Skilled Nursing Facility.Shift: Days.Shift Hours: 0800-1630.Work Days: Monday, Tuesday, Wednesday, Thursday, Friday.Meal Break: 30 minutes.Contract Length: 4 weeks.Guaranteed Weekly Hours: 35.
