Travel LCSW - Full Time Contract

Make a meaningful impact as a Licensed Clinical Social Worker in Poughkeepsie, NY. We seek an LMSW or LCSW with acute care discharge planning and case management experience to support inpatient and outpatient coverage at a hospital facility. Advocate for patients to obtain needed resources and coordinate transitions of care in a dynamic clinical environment. Minimum Qualifications Master’s degree in social work (MSW) from an accredited program.Current NY State LMSW or LCSW in good standing. (Client requires NYS LMSW or LCSW.)Minimum 3 years of acute care hospital discharge planning experience, including transitions of care and case management duties.Experience with Cerner electronic health record.Experience with WellSky (or equivalent care coordination/placement systems).Ability to perform inpatient and outpatient discharge planning tasks such as skilled nursing facility placement, home care coordination, durable medical equipment arrangements, transportation coordination, insurance authorization support, family communication, and discharge education.Internal floating within the case management team to cover multiple inpatient and outpatient areas is required.Preferred: experience with RoundTrip, Parachute, or payor portals. Assignment Details Facility type: Hospital.Shift: Days.Shift hours: Days 07:00-15:30.Schedule: Monday–Friday with every 4th weekend and rotating holidays per client scheduling.Position type: Travel assignment, 13-week duration.
