Minimum Requirements Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program (required).Current State License as a Physical Therapist in Kentucky or ability to obtain prior to start (required).Proof of current CPR certification (required).Current driver's license, reliable transportation, and vehicle insurance (required).1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting (required).Previous home health experience (preferred).Ability to perform prolonged standing, walking, lifting, bending, pushing, and pulling as required for in-home care (required).Ability to work a flexible schedule and travel locally (required).Demonstrated knowledge of durable medical equipment and ability to order/adapt equipment for home environments (required).Ability to read, write, and speak fluent English (required). Assignment Details Facility type: Home Health.Assignment type: Travel / Temporary contractor.Candidate must collaborate with interdisciplinary teams and coordinate with physicians as needed.Must provide patient evaluations, develop and revise Plans of Care, document progress notes, and process phone orders.May be required to educate patients and families on transfers and use of assistive devices in the home.Must be willing to perform job duties consistent with education and competence and accept assigned caseloads in the home health setting.
