Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.Active Kentucky Physical Therapist license is required to start the assignment.BLS (Basic Life Support) certification from AHA or ARC may be required.Home health experience preferred. Candidates with prior experience conducting evaluations, developing home exercise programs, and delivering skilled in-home interventions will be given preference.Experience with common home health documentation systems and electronic medical records preferred.Ability to travel within an assigned geographic caseload and reliably provide patient visits in home settings.Reliable vehicle and valid driver’s license recommended for travel between patient homes.
