Position Title: Full -Time Mentor Department: Juvenile Justice-Mentoring & Advocacy Program (MAAP)Reports To: MAAP Program SupervisorIs this the next step in your career Find out if you are the right candidate by reading through the complete overview below.Job Summary: The MAAP Mentor is responsible for working with high risk youth throughout the borough of Brooklyn between the ages of 12 to 21.Mentors are responsible for providing individual mentoring, case management and advocacy for youth on their respective caseloads.Mentors should be familiar with issues facing inner-city high-risk youth.Mentors must also be familiar and comfortable traveling and connecting to social, recreation and youth resource supports throughout Brooklyn as well as in the Brooklyn neighborhoods where their mentees reside.He/she will be tasked with creating individualized goals plans and providing direction and support to youth around completion of those goals.Mentor will attend school and program meetings as necessary to advocate for youth.Mentors will make referrals and provide monitoring for outside services when needed.Mentor will engage in social activities with youth, with the goal of creating a strong mentor/mentee relationship in order to accomplish aforementioned tasks.Mentors will also be responsible for crisis intervention and more intensive case work and mentoring for high-risk cases.All Mentor activities will be under the direction of the MAAP Social Worker and Program Supervisor.*COVER LETTER IS REQUIRED* Major Duties & Responsibilities:1.Carry an assigned active caseload of 5 high risk youth, referred by DYFJ at all times.2.Develop strong rapport with all youth on caseload based on a strength based perspective.3.Conduct intake and clinical assessment of assigned youth, in conjunction with program social worker.4.Develop, alongside youth, individualized success plans, which include goals, ideas, dreams and aspirations for each youth on caseload.5.Offer one to one mentoring support to help youth develop sustainable relationships with family, peers, school and community.Home and school visits required.6.Responsible for obtaining ongoing school and medical records for youth on caseload.7.Provide mentoring services with intensive supervision and 24/7 crisis intervention for youth on caseload.8.Work with youth and family to safely and successfully address issues of conflict and stress within the family and home environment.9.Assist youth and family to fully understand and adhere to all court ordered obligations, conditions of probation or aftercare.10.Appear at court hearings and at probation or aftercare meetings to support youth, and help to articulate efforts, progress and obstacles encountered in pursuing identified goals and adhering to required conditions.11.Provide direct engagement with youth around pro-social activities and family engagement activities and supports.12.Provide and/or connect youth to services that will increase youth engagement in education, employment, community service as well as improve their overall social and emotional...
