Job Title: RN CoordinatorProvidence Home Services is seeking a skilled RN Coordinator to join our team. As a key member of our nursing staff, you will be responsible for planning and coordinating new referrals, providing phone triage to existing patients, and delivering direct nursing services and case management related to infusion therapy.Interested in learning more about this job Scroll down and find out what skills, experience and educational qualifications are needed.Key Responsibilities:Plan and coordinate new referrals, ensuring seamless transitions for patients and families.Provide phone triage to existing patients, addressing their concerns and needs in a timely and compassionate manner.Deliver direct nursing services and case management related to infusion therapy, working closely with patients, caregivers, physicians, and payors to ensure the best possible outcomes.Requirements:Oregon Registered Nurse License or Washington Registered Nurse License upon hire.Within 30 days of hire, National Provider BLS - American Heart Association certification.Must be licensed in states wherever care is delivered.3 years Home Infusion nursing experience.Preferred Qualifications:Bachelor's Degree in Nursing.Why Choose Us?At Providence Home Services, we offer a comprehensive benefits package designed to support your well-being, growth, and financial security.
