**This is an In Office position**Interested in learning more about this job Scroll down and find out what skills, experience and educational qualifications are needed.This role operates in a Day Habilitation (DAY HAB) Setting, defined as services that provide opportunities and support for community inclusion and build interest in and develop skills for active participation in recreation, volunteerism and integrated community employment (Iowa Health & Human Services). Day Habilitation provides assistance with acquisition, retention, or improvement of socialization, community participation, and daily living skills. Community Integration Coordinators support the mission of the organization empowering people to reach individual achievement across the spectrum of life. As a Community Integration Coordinator, youll provide leadership to Community Integration Professionals, Environmental Associates, General Laborers, Production Associates, Clerical Assistants, and Assemblers. As an integral part of the overall supported employment team, you will collaborate with team members, individuals-served, and their families to ensure the best possible care for those you serve. You will creatively strategize ways for an individual to find employment that suits their skills, abilities, and goals. In this role, youll be responsible to:Create and maintain the team culture, which may include:o Collaborating with Imagines' Recruitment Team to hire the best team members for the job.o Supervise Community Integration Professionals, Environmental Associates, General Laborers, Production Associates, Clerical Assistants, and Assemblers.o Collaborating with Imagines Training Team to provide the best possible training for team members, assuring they learn the most updated and efficient practices.o Scheduling your team to work at times that fits the needs of the people we serve.o Being actively present and involved with your team, including performing weekly site visits and following up with team members.o Conducting monthly team meetings focused on immediate needs and team culture.o Providing coaching opportunities for staff.o Agreeing to be placed on-call as required and fill in for the team if there is need.o In this role, you must be available on short notice to fill gaps in the schedule due to absences, personnel changes, or other unexpected reasons.o Compensation for on-call duties will be paid in stipends based on rotation, per department guidelines.o Mileage reimbursement is available when on-call duties require report to work outside of regularly scheduled hours.Overseeing resources, which may include:o Monitoring and scheduling vehicle usage for services.o Monitoring budgets for individuals served are being maintained and followed as outlined in their financial plan by the Representative Payee Coordinator.o Supporting individuals in communication with and fulfilling responsibilities associated with their Representative Payee Coordinator.o Overseeing balances, ledgers, receipts, and special requests forms in collaboration with the Representative Payee Team.o Monitoring medication check-ins.o Monitoring appointment scheduling and follow-up communication for individuals served.Ensuring services are provided to individuals in a way that honors their needs, desires, and goals, which may include:o Serving as Imagines' primary point of contact with the Medicaid Community Based Care Manager (CBCM) for individuals served.o Inputting accurate information regarding the individual served into the approved Electronic Medical Record (EMR).o Developing individualized plans with the CBCM and Interdisciplinary Team (IDT) that prioritizes the individuals needs and goals.o Updating essential documents and ensuring authorizations are accurate. In addition, this role may require assessments to be completed for specific services or individuals.o Conducting quarterly reviews of goal progress for each individual-served under your teams care.o Communicating with individuals served, their families/guardians, and case managers in a timely manner regarding any changes in the services provided. This includes changes to the kinds of services provided, a limitation of rights, or other concerns regarding the individual served. In this role, you will also be responsible to document these interactions and reasons for changes.o Collaborating with the IDT to write support goals and a personal outcome plan within 21 calendar days of a new individual being entrusted to our care.o Ensuring resources, such as ongoing supports, transportation, or other vital components to services provided, are available.o Collaborate with the Strategic Advancement team to promote local community partnerships and promote services offered by Imagine.Demonstrate knowledge, or be willing to learn, and comply with all policies, practices, laws, and rules provided by any governing bodies in the industry.Provide complete, consistent, and accurate documentation of incident reports, investigations, and service documentation.Be an advocate for individuals receiving services to have as much control over their own lives as possible.
