This position is responsible for conducting face-to-face member assessments in the McAllen/Edinburg area and ensuring compliance with State and Company guidelines. The role requires strong case management skills, pediatric experience, and the ability to manage complex member care needs. Responsibilities Conduct in-home member assessments 3 days per week and complete documentation 2 days per week. Complete face-to-face visits and submit all required assessments and forms within turnaround times. Perform outreach to members as needed. Evaluate service needs of high-risk or high-acuity members and develop care plans for best outcomes. Ensure compliance with State, Company, and regulatory requirements. Collaborate with team members and internal partners to coordinate care. Candidate Profile Required: Graduate from an accredited school of nursing OR Bachelorí¢â‚¬â„¢s degree with 4í¢â‚¬"6 years of related experience. Active RN (Registered Nurse) or NP (Nurse Practitioner) license in Texas or compact state. RUG (Resource Utilization Group) certification required. 2+ years of Case Management experience. 2+ years of Pediatric experience. Strong background in healthcare compliance and documentation.
