This position is responsible for conducting face-to-face member assessments in the McAllen/Edinburg area and ensuring compliance with State and Company guidelines. The role requires strong case management skills, pediatric experience, and the ability to manage complex member care needs.ResponsibilitiesConduct in-home member assessments 3 days per week and complete documentation 2 days per week.Complete face-to-face visits and submit all required assessments and forms within turnaround times.Perform outreach to members as needed.Evaluate service needs of high-risk or high-acuity members and develop care plans for best outcomes.Ensure compliance with State, Company, and regulatory requirements.Collaborate with team members and internal partners to coordinate care.Candidate ProfileRequired:Graduate from an accredited school of nursing OR Bachelor's degree with 4-6 years of related experience.Active RN (Registered Nurse) or NP (Nurse Practitioner) license in Texas or compact state.RUG (Resource Utilization Group) certification required.2+ years of Case Management experience.2+ years of Pediatric experience.Strong background in healthcare compliance and documentation.Preferred:Bachelor's degree in Nursing.Home Health experience.Disqualifiers:No Home Health experience.Top Skills:Case Management (2+ years)Pediatrics (2+ years)Home HealthWhy Join Us?Direct impact on member care through in-home visits and service planning.Gain valuable field experience in pediatrics and case management.Be part of a mission-driven team ensuring high-quality care for members.Opportunity for extension or conversion based on performance.PandoLogic.
