Employment TypeIf you think you are the right match for the following opportunity, apply after reading the complete description.Full timeShiftDescription:Position PurposeOur home care LPNs, Licensed Practical Nurse, use cutting edge technology and clinical knowledge to provide exceptional care to patients who require one-to-one attention and monitoring in their home. Home care LPNs are responsible for client assessment, care planning, intervention and overall supervision of patient care and outcomes. Our nurses follow physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.What You Will DoSupport Trinity Health at Home's mission, values, strategic goals and high standards of customer service.Adhere to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.Responsible for ensuring outstanding customer service for all infusion services.Responsible for overall operation profitability.
