This role supports patients with complex wounds through assessment, treatment, education, and collaboration with interdisciplinary teams, while also contributing to quality improvement initiatives and staff education.Key ResponsibilitiesProvide expert wound, ostomy, continence, fistula, and complex wound care servicesAssess, treat, and document patient responses to wound care interventionsConduct comprehensive skin and pressure injury risk assessments and implement prevention strategiesDevelop, revise, and evaluate wound care standards and best practicesAssist in the selection and evaluation of wound care supplies, equipment, and technologyEvaluate the quality and delivery of wound care provided by clinical staffCollaborate with physicians, nursing staff, and interdisciplinary teams to coordinate careEducate patients and families on wound care, ostomy management, continence care, and prevention strategiesProvide staff education, including new employee orientation and in-service trainingParticipate in unit-based and organization-wide quality improvement initiatives, including prevalence and incidence studiesCommunicate and escalate clinical challenges, ethical concerns, and conflict resolution issues as appropriateSupport culturally competent care and education tailored to diverse patient populationsMaintain confidentiality, data security, and compliance with patient rights and regulatory standardsTrack and manage wound care-related equipment, including specialty beds and negative pressure wound therapy devicesEngage in ongoing professional development to maintain clinical competencyIdentify community resources and assist patients in accessing post-discharge support servicesRequired Experience & Education xijylhu Associate Degree in Nursing (ASN or ADN) from an accredited nursing programMinimum of 2 years of Med/Surg nursing experience with a strong focus on wound and ostomy careRequired Licensure & CertificationsActive New York State Registered Nurse (RN) LicenseBLS (AHA) certificationOne or more of the following wound care certifications:Certified Wound Ostomy Continence Nurse (CWOCN) - WOCNCBCertified Wound Ostomy Nurse (CWON) - WOCNCBCertified Wound Care Nurse (CWCN) - WOCNCBCertified Wound Specialist (CWS) - ABWMWound Care Certified (WCC) - NAWCOFor more details reach at or Call / Text at 516-587-6677.
