Job DescriptionIncrease your chances of reaching the interview stage by reading the complete job description and applying promptly.The Advanced Practice Provider (APP) supports the clinical, educational, and research missions of the organization. This role may provide care in inpatient and/or outpatient settings, via telehealth, and through triage. The APP performs regular health assessments, diagnoses and manages acute and chronic conditions, and develops treatment plans that include medical management, psychosocial support, health education, and counseling. The APP collaborates with physicians and other health professionals to assess and manage health problems, maintain continuity of care, coordinate community services, and contribute to the ongoing development and improvement of the health care delivery system.Primary Clinical ResponsibilitiesProvide care for adult patients in the Department of Medicine Electrophysiology Clinic.Perform new and follow-up patient visits:Gather data from initial evaluations and medical records.Review outside medical records and communicate with referring providers as needed.Develop, document, and implement an assessment and plan of care.Generate and document progress notes for each outpatient visit, ensuring problem lists are kept up to date.Maintain consistent communication with referring physicians and other health care providers.Coordinate care with the cardiac device clinic, including providing coverage for the device clinic when needed.Serve as an expert resource for providers and patients regarding cardiac electrophysiology questions or issues.Generate and execute evaluation and management plans in urgent situations.Perform clinic-based procedures, including:Implantation of implantable loop recordersCardioversionEssential DutiesPatient Care & Clinical ManagementProvide high-quality care for patients and families.Manage both chronic and acute conditions and oversee health maintenance and health promotion.Perform and document comprehensive histories and physical exams.Monitor and document patient progress, revising the plan of care as needed.Interpret and monitor laboratory and diagnostic test results.Identify issues requiring specialty referral or escalation to a higher level of care and intervene appropriately.Communication, Education & Community EngagementAct as a consultant and provide current educational programs to community groups, organizations, or individuals upon request.Contribute to the education and research mission of the organization through:Participation in research or quality improvement projectsPresentations and involvement in professional organizationsPrecepting and other scholarly activitiesProfessionalism & LeadershipFunction as a professional role model and encourage professional behaviors in interactions with patients, families, significant others, and members of the multidisciplinary team.Represent advanced practice, the Department, or Division on appropriate organizational committees.Regulatory Compliance & LicensureAbide by all applicable National Jewish policies.Comply with all laws, rules, and regulations governing advanced practice, including those set by:Colorado Medical BoardThe Joint CommissionMedicare and Medicaid and other relevant agenciesMaintain current:State license (Colorado PA or APRN) and prescriptive authorityBoard certificationBLS and/or ACLS or PALS certification appropriate to clinical responsibilitiesStay current with changes in regulations and standards of practice.Diversity, Inclusion & SafetyFoster an inclusive workplace where diversity and individual differences are valued and leveraged to achieve the institutions vision and mission.Adhere to safe working practices and follow all institutional and departmental safety policies and procedures.Wear appropriate personal protective equipment (PPE) as outlined by infection control policies and procedures.Demonstrate compliance with all state, federal, and other regulatory agency requirements.Minimum QualificationsEducationGraduate degree from an accredited Physician Assistant or Nurse Practitioner program required.Work Experience1 year of recent, related experience as an Advanced Practice Provider preferred.Experience in Cardiology and Cardiac Electrophysiology preferred.Licensure, Certification & TrainingCurrent Colorado PA license or APRN license with prescriptive authority.DEA Registration.PA or NP certification by a national certifying body.Current CPR/BLS certification required.ACLS or PALS certification required if applicable to the specific practice area. xijylhu Certified Cardiac Device Specialist (CCDS) certification preferred.
