Job DescriptionTrinity Health is seeking a compassionate and skilled Advanced Practice Provider (APP) to join our Family Medicine team. The ideal candidate will play a vital role in providing high-quality care to patients of all ages, promoting wellness, and managing both acute and chronic health conditions. As an integral part of our family medicine practice, the APP will work alongside physicians and other healthcare professionals to ensure comprehensive care for our diverse patient population.Key Responsibilities:Provide primary and preventive care to patients across the lifespan, including children, adults, and elderly.Conduct comprehensive physical exams, obtain medical histories, and perform diagnostic tests.Diagnose, treat, and manage common acute and chronic medical conditions.Prescribe medications, order treatments, and provide patient education on disease prevention, treatment plans, and lifestyle modifications.Manage patient follow-up care and ensure continuity of care across various settings (inpatient, outpatient, etc.).Qualifications:Please make sure you read the following details carefully before making any applications.Licenses and Certifications RequiredCurrent ND Board NP/PA License required.Cardio Pulmonary Resuscitation (CPR) training, with certification every two (2) years, preferred.ACLS certification, with certification every two (2) years, preferred.Educational RequirementsEducation as required to obtain and keep current ND NP/PA license.Experience RequirementsExtensive knowledge of established professional medical concepts, principles and practices, required.Experience in specialized medical areas, preferred.CPT coding knowledge, preferred. xijylhu If you are a dedicated family medicine advanced practice provider looking to make a significant impact on the lives of patients, we invite you to apply!
