This role supports two cardiologists in a growing outpatient practice and is essential to improving patient access and care continuity.---About the OpportunityThe NP will manage follow-up visits for established patients, allowing cardiologists to focus on new referrals and complex cases. The NP will also support transitional care efforts and provide timely post-discharge visits for patients with heart failure.
