Family Medicine Physician - Community Health Center , Bay Area About the OrganizationThis non-profit Federally Qualified Health Center (FQHC) serves communities across the East Palo Alto, Palo Alto, Mountain View, and Sunnyvale areas.The organization provides integrated primary, preventive, behavioral health, and social services to a diverse and underserved patient population.Position SummaryThe Family Medicine Physician provides comprehensive, patient-centered primary care to patients across the lifespan in a community health setting.This role focuses on delivering high-quality care in a team-based, mission-driven environment that addresses both medical and social determinants of health.Key ResponsibilitiesProvide full-scope outpatient primary care, including preventive, chronic, and acute careDiagnose and treat a wide range of conditions across pediatrics, adults, and geriatricsCollaborate within a team-based care model including medical assistants, behavioral health providers, and care coordinatorsParticipate in care coordination and population health managementMaintain accurate and timely electronic health record (EHR) documentationSupport and engage in quality improvement initiativesDeliver culturally competent care to a diverse patient population QualificationsMD or DO degree from an accredited institutionBoard certified or board eligible in Family Medicine (or Internal Medicine)Active or eligible for California medical licenseExperience in community health, FQHC, or underserved settings preferredBilingual in Spanish strongly preferred Practice EnvironmentMission-driven FQHC focused on underserved populationsIntegrated care model including medical, dental, behavioral health, and social servicesDiverse patient population including Latino, African American, and Pacific Islander communitiesEmphasis on whole-person, xijylhu patient-centered careSupport staff includes two medical assistants per provider, with approximately 80% Spanish-speakingProductivity expectation of approximately 10 completed appointments per 4-hour session Additional HighlightsTeam-based model with strong medical assistant supportOpportunities for loan repayment programs, including NHSC and PSLF eligibilityFlexible FTE options ranging from 0.5, .08 or 1.0Mix of telehealth and in-person visitsFTCA malpractice coverageSalary range- $240K- 325K$10,000 sign-on bonusRelocation assistance up to $7,500 for out-of-state candidates and $5,000 for in-state candidates
