RN/LCSW Care Manager

Full-Time Case Manager openings in the Rocky Mount, NC area! MUST have RN or LCSW license! This role is ideal for an RN or LCSW who likes to travel! Mileage and Lodging is reimbursed Pay: $38-$42/hr based on experience Schedule: Monday - Friday 8am-5pm EST Location: Candidates MUST reside in the Rocky Mount, NC area and can travel up to 2.5-3 hr radius from home residence for face-to-face member visits. Mileage is reimbursed and lodging is reimbursed if needed! Job Responsibilities: Managing a case load for healthcare members with long term care needs. Monthly and quarterly member contact and will include 80% travel. Remote role. Will require a driver's license. Member assessments and notes. Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development. Monitor delivery of services and follow-up with members, caregivers, or provider s through in person visits and telephonic contact Authorize and coordinate referral for services. Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care. Assist in coordinating the development of informal or voluntary services to integrate into the member care plan Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plan, care plan, and coordination of acute care and long-term care services! Assist member with filing and resolving complaints and appeals. Required Experience/Education: 2+ years of Case Management; Case Management with a health plan is HIGHLY preferred North Carolina RN or LCSW license with no restrictions Bachelors Degree Experience with Electronic Medical Records Good computer skills with Microsoft Office (Teams, Word, Outlook, Excel, etc) Preferred Skills: Long term care Discharge planning Apply now for immediate consideration with Silvana Manzella or send resume to
